In order for a course to be certified, it must be measured using the Calibrated Bicycle Method. The following information must be included:

Though the Canadian forms are not yet downloadable, you may use the USATF forms.

The application forms needed for USATF/RRTC course certification are all provided in an Adobe PDF (Portable Document Format) file named "appforms.pdf" which you can download from this page. The forms contained in this file are:

To read and print these application forms, you'll need the (free) Adobe Acrobat Reader software. Use the following link to download the Acrobat Reader software if you don't have it already:

The PDF file containing application forms is downloaded using the link below. Depending on how your browser is configured, selecting that link the usual way (e.g., by left-clicking) may open the PDF file directly in your web browser (using optional Acrobat browser plug-in). You can print application forms this way, but we don't recommend it because you shouldn't have to go online every time you want to print an application form! The best approach is to instruct your browser to download the appforms.pdf file to your hard disk without displaying it in the browser window at all.

To force your browser to download appforms.pdf to your hard disk, use the following trick: If running Windows or Unix, right-click on the link below for appforms.pdf. Or, if using a Mac, click on the link and hold the mouse button for at least about a second. When the pop-up menu appears, choose "Save Link As..." or "Save Target As..." or "Download Link to Disk" (the wording depends on which browser you're running). Save the file to a convenient location on your hard disk. If the dialog box gives you a choice of format, choose "Source" format. Then, whenever you wish to print an application form, you don't have to be online; just locate the downloaded appforms.pdf file on your hard disk and double-click it.

Note: You'll probably have to include a processing fee when sending the filled-out application forms to your regional certifier. The exact amounts of these fees may vary. Therefore, check with your certifier before sending in the application forms.
