Private Insurance

Many companies now offer coverage for chiropractic care. Check your policy!
We will bill insurance companies directly provided they have that option.

We need to know:
1. If we can bill your insurance company directly.
2. The company name.
3. The group or contract number.
4. Contact or employee number.
5. The subscribers name.
6. Amount of deductible (if there is any)
7. Percentage of treatment covered (eg. 80% or $350)
8. Do you have acupuncture (if covered and dollar amount)
9. For your convenience, please provide us with copies of your insurance forms.
