If you are interested in moving into Alpha House, but would like to speak to someone before making the decision, please call our office at (204) 982-2011. REFERRAL FORMS AND APPLICATION PROCESS If you feel like you have enough information and would like to apply, please print off, complete, and return the forms below via fax or mail. Please do not email your application form.
Both forms are preferred and assist in the application process, but if you do not have an agency currently working with you please do not hesitate to fill out the applicant referral form and send it in. The second step, after your application has been received and reviewed, is an interview. This provides an opportunity for the Director to meet directly with you, the applicant, to further understand your story and decide if our services will benefit you.
